Communication as a basis for therapeutic relationships through the process of assessment, care planning, delivery and evaluation in the context of first contact, continuing care and public health and health protection
Assessing health needs of individuals/families to include risk and needs assessment in the context of first contact, continuing care and public health and health protection
Collaborative working across the nursing and social interface incorporating service users and providers
Knowledge and skill development for specialist clinical practice in relation to the DN/HV/PN/SN/OHN

Pathway Specific Syllabus
Relating HV practice, particularly primary prevention to human development across the life span
Establishing effective relationships with individuals, families and community groups and also with other members of the primary health care team and statutory and voluntary agencies
Targeting health surveillance within the context of first contact, continuing care and public health and health protection incorporating assessment of risk and secondary prevention
Applying the principles of Health Visiting to working with individuals, families and communities
Considering the public health and health protection role of the Health Visitor
Case study of family centred public health in relation to nutrition
Orientation and articulation of nursing in the general practice setting
Application of interpersonal communication to facilitate patient/client decision making
Analysis and evaluation of the effectiveness of consultation skills within the general practice setting in relation to patients/clients and also with other members of the primary health care team and statutory and voluntary agencies
Development, application and evaluation of the use of a variety of models of appropriate intervention at primary, secondary and tertiary levels of care for individuals, families and groups registered with the general practice
The use of patient/client centred approaches to devise appropriate care and chronic disease management programmes, integrating evidence based knowledge within the primary care setting
Communication in the context of decision making regarding admission and discharge of patients to/from case load; on-going management of patient/family care in domestic, clinical and residential care settings; creating and maintaining appropriate relationships with patients, families and colleagues throughout the process of care
Assessment of health needs to inform provision of a district nursing service that is responsive to the health and wellness needs of different patient/family groups in the context of first contact, continuing care and public health; liaising and co-ordinating with social services and other agencies to identify needs and appropriate service provision; identify individual health potential and risk of breakdown or deterioration of current health state
Integration of theory and evidence based knowledge to devise and deliver patient/family centred care and complex care programmes taking into account cultural differences of individuals and groups represented within a case load/community
Selection and critical integration of theoretical models within the context of a range of district nursing interventions and contexts.
Application of the principles of HV/SN to work with individual children, groups, families and the school/community
Application and evaluation of theories/models of interpersonal communication to contexts of health interviews with children/parents/carers and establishing effective relationships with other colleagues in the primary health care team, school and other statutory and voluntary agencies
Health needs assessment (including ‘first contact’) in the context of individual children, their families, peer groups and the school/community
Programmes of health surveillance and intervention for the school age population
Interaction of health and development in school age children and their families; continuing care of vulnerable children and groups included looked after children; the extended school;
Public health case study of the child in the health promoting school in relation to nutrition; The National Healthy School Standard in practice.
Occupational Health Nursing
Current legislative and policy influences upon the development of occupational health services and practice for individuals, families, communities and organisations;
Targeting of resources to vulnerable individuals and groups in the community workplace
Frameworks for assessment and monitoring of individuals and groups in relation to workplace health and wellbeing, human development and levels of prevention;
Managing opportunities and risk in the interface between employees, employers, the health care system and the wider community;
Higher order therapeutic skills for the engagement of individuals groups in the planning and management of care
Application of theoretical and evidence base to occupational health and safety as public health practice: the principles of specialist community public health nursing.
100 hours practice based learning
Learning contracts will focus on areas of specialist practice and/or the principles of health visiting as appropriate.
100 hours college based learning
Classroom sessions - 35 hours (14x2.5)
This will include lectures and case studies. Practical sessions will relate to consultation/assessment/communication skills with application to the pathway context. Specific topics will be supported by enquiry/problem based learning undertaken in Enquiry Based Community Practice.
The assessment will take the form of practice based course work and formative assessment of practice.

|Description||An analysis of the principles underpinning your specialist practice, usnig case study material, demonstrating the application of higher levels of communication skills in the assessing, planning and implementation of health care. 4,000 words||Formative assessment in practice. Reflecion on expereince in practice and campus based learning as a resource for completion of assignment, demonstrating the acquistiton of skills defined within the four areas of specialist practice.|
